Cancellation and Rescheduling Policy
We require a minimum of 2 hours’ notice before the appointment time to cancel or reschedule your appointment. If you fail to give less than 2 hours’ notice a DNA invoice for $100.00 will be invoiced
Medical File Transfer
If medical files are transferred out of the practice, there is an administration fee to cover the cost of the paper/ disk and administration time. Full transfer starts at $50 + GST the records will be faxed or copied and sent by registered post. Please note a Patient Health Summary request has no fee
